For this assessment criterion, you will be required to demonstrate that you have ensured the implementation of an individual’s support plan.
As a lead practitioner, you will need to ensure that staff are providing support in line with the care plan and that you are available to answer any questions or respond to any concerns that they may have. You may also be responsible for the staff rota and ensuring that enough staff are available to support the individual as required.
This could also involve getting regular feedback from the individual, their family and staff to monitor how the support is being provided and any changes that may need to be made. For example, an elderly individual with dementia may require more support over time, whereas a young adult with a learning disability may require less support over time as they learn new skills.
