Ensure implementation of a support plan.

This page is designed to answer the following questions:

NOTE: Although this page has been marked as complete, it has not yet been peer-reviewed or quality-assured, therefore it should be considered a ‘first draft‘ and any information should be fact-checked independently.

For this assessment criterion, you will be required to demonstrate that you have ensured the implementation of an individual’s support plan.

As a lead practitioner, you will need to ensure that support staff are providing support in line with the care plan and that you are available to answer any questions or respond to any concerns that they may have.

This could also involve getting regular feedback from the individual, their family and staff to monitor how the support is being provided and any changes that may need to be made. For example, an elderly individual with dementia may require more support over time, whereas a young adult with a learning disability may require less support over time as they learn new skills.
