Your organisation will have systems and processes for recording any changes to an individual’s care plan following a review. The system used should maintain a record of previous iterations of the care plan to maintain a proper audit trail.

For paper-based systems, this may utilise a ‘Review Page’ at the front that logs the date of the review and any changes to the support plan. Changes to care plans may be recorded automatically on digital computer systems, as well as having the option to revert back to older revisions.

It is essential that you follow organisational agreed ways of working for the documentation of care plans and care plan reviews so that proper legal and regulatory records are maintained. Following procedures also ensure that information remains confidential and secure in line with the Data Protection Act 2018 (and the General Data Protection Regulations).
