For this assessment criterion, you will be required to demonstrate that you are able to record the care plan review process and outcomes according to organisational systems and procedures to support information sharing.

Your organisation will have systems and processes for recording any changes to an individual’s care plan following a review. The system used should maintain a record of previous iterations of the care plan to maintain a proper audit trail.

It is essential that you follow organisational agreed ways of working for the documentation of care plans and care plan reviews so that proper legal and regulatory records are maintained. Following procedures also ensures that information remains confidential and secure in line with the Data Protection Act 2018 (and the General Data Protection Regulations).
