Record a plan according to organisational systems and processes to support information sharing

This page is designed to answer the following questions:

NOTE: Although this page has been marked as complete, it has not yet been peer-reviewed or quality-assured, therefore it should be considered a ‘first draft‘ and any information should be fact-checked independently.

For this assessment criterion, you will be required to record a care and support plan according to organisational systems and processes to support information sharing.

Having agreed upon a plan of support with an individual, it is essential that it is documented so that all care staff understand the needs of the individual and how to support them correctly and consistently.

The method of recording a care plan will be dependent on your organisation’s policies and procedures. Although many care providers now use digital recording systems, there are still some that use a paper-based system. Whichever is used, security and confidentiality will be paramount and you must follow your employer’s agreed ways of working to ensure that it s only accessed by authorised persons.

Care plans should be easily accessible by care staff and a consistent structure and format should be used so that information is easily located and retrievable.
