Having agreed upon a plan of support with an individual, it is essential that it is documented so that all care staff understand the needs of the individual and how to support them correctly and consistently.

The method of recording a care plan will be dependent on your organisation’s policies and procedures. Although many care providers now use digital recording systems, there are still some that use paper-based systems. Whichever is used, security and confidentiality will be paramount and you must follow your employer’s agreed ways of working to ensure that it s only accessed by authorised persons.

Care plans should be easily accessible by care staff and a consistent structure and format should be used so that information is easily located and retrievable.

It is also important to explain to individuals how their personal information will be shared with others and to obtain their consent.
